I think the story is going to split up. Instead of the story focusing on Team Konohamaru / Konohagakure, I think later down the line Boruto is going to join Kara (for unexplained reasons) and there will be a split focus.
People keep asking where Kawaki is, and the first chapter hinted that Boruto and Kawaki have some kind of relationship in which they know each other. Like Boruto, Kawaki has the same seal, and the newest organization has a member who also has the same seal. Jigen. So there is a correlation somewhere.
We at first interpreted that Kawaki must have killed an Otsutsuki to gain the seal, however, Jigen having the same seal as them both would mean they all killed Otsutsuki? Wrong. I'm pretty sure Boruto is the only one who received a seal directly from killing Momoshiki, and rather KARA are either creating their own, or they are being guided (or even misguided) by Otsutsuki themselves (or information the Otsutsuki 'gave' them cough).

My theory is that Bolt will infiltrate Kara as a spy for Konohagakure, whether the narrative of the story makes his actions clear (as in him being a double agent) or they might make it seem as if he actually went 'rogue'. It would make a nice contrast with his Master Sasuke (and even Itachi perhaps since Boruto wishes to become a building block for the Leaf in the shadows). He could then support Konohagakure from afar.
We've had this situation before (with Kabuto and Itachi for example), but never in the perspective of the Main Character It's also possible that the KARA might not be exactly total bad guys. They might be netural but just have conflicting views on the shinobi world, for instance.

Reasons as to why Boruto might go to KARA:
- Sasuke can't help Boruto control his seal. He knows little to nothing about his seal, and despite warning Boruto to come to him if something happens, he hasn't shown the knowledge via fuuinjutsu in the same way Kakashi did years ago.
Per example:
- Someone gets hurt.
Momoshiki already warned Boruto (and the readers) that Boruto's entire person holds a risk. And I can't help but think that Boruto's seal is going to signal him out in a bad way. It's only a matter of time before something or someone gets hurt because of the power Momoshiki bestowed on him. If that happens, it would make sense for Boruto to run away to Kara so that he doesn't kill anyone important to him.
- Boruto becomes a double agent.
The most obvious one. Boruto (with the help of Sasuke and the others) goes on a secret mission in which he looks like he betrays the Leaf, and he joins Kara. He works as a double agent and secretly spies on them. If you're asking why Kara would even care about Boruto, it's probably because according to Shojoji, they were discussing the same Curse Mark that Boruto has on his hand. Not to mention Jigen himself has the same mark on his Chin. It's only a matter of time before KARA find out about Boruto, and when that happens, contact will be imminent.
