hagoromo used izanagi many times with his rinnegan, and yet he didn't lose his sight
Could Sasuke have used Izanagi at VOTE 2?

Yes it would have as Sasuke would have had both arms and capable of fighting still.He had EMS and Rinnegan and he couldn't kill Naruto. What the hell is he going to do with just a Rinnegan?
If he activated Izanagi as a last resort when him and Naruto clashed. Sasuke would still have both his arms and Naruto would be in the condition he was in
Sasuke coule simply use the non rinnengan eye to activate Izanagi. All it requires is 3T
As for Izanami , you need to hit the enemy two times exactly similar .... I don't think that's gonna happen against Naruto .
Izanagi is a jutsu made to survive death .... Sasuke didn't need this jutsu at VOTE 2 .

Now the Question you should be asking yourself is, does he know how to use the technique?
What would he do afterwards? He was low/hardly had any chakra left. Say he uses the technique and somehow kills Naruto, then what? He'd still be in the same condition that he was in just with both arms and this Naruto ain't alive to stop Kakashi from killing him. There goes your fanfic scenario, anyway you look at it Sasuke will always come out the bitter loser in the end.
Izanagi can just make you healthy ; it won't bring back all the chakra you lost ...
So , if Sasuke used Izanagi back then , he would have his other hand ; but had no chakra (He almost put all his chakra in his last attack) .
He used creation of all things to create the Bijuu. Izanagi falls under the imagination part of creation of all things. Hagoromo used the Yin half to create something out of nothing and then the Yang half to give it life. Izanagi only uses Yin to create something out of nothing, in this case a Genjutsu that rewrites reality (to a degree), which costs them an eye. It's not the same thing.
Edit: Just looked it up it uses Yang release to bring the imagination to life. Still it falls under the category of Creation of all things, but what the Sage used was not Izanagi.

After they are lying down Izanagi activates and Sasuke still standing. He may be low on chakra but Sasuke would be able to finish an incapacitated Naruto OR just leave him there to die

nah bro, izanagi is yin-yang release jutsu, cuz you need both senju and uchiha powers to perform it

He was already quite beat up before their last clash and both of them fainted. Sure he could've activated Izanagi before but for him to rewrite the reality he needs to be conscious after activating it or set a timed activation to restore himself which I doubt he knew how to do. In case of the latter he at least need to know when to time and activate it and without him knowing the outcome of their last clash he'd be putting his eyes at a huge risk. So again doesn't makes sense.
The only time using Izanagi makes sense is when the user is about to die otherwise there's a huge risk and uncertainty of sacrificing the eye for nothing.
I think izanagi needs a little chakra to be activated .
Sasuke was so confident that his last jutsu would finish Naruto . So he put all his chakra in that last jutsu .
I think he didn't have enough chakra for preforming izanami .
He could if his only goal was to kill Naruto and then stop fighting. Yet his plan was to keep fighting after defeating Naruto and he would need his both doujutsu to be a real threat to a shinobi world like Obito or Madara were, so all the nations could stay united.

He swallowed it yes but hashi's dna/chakra hadn't mixed with his own when he used izanagi.
Otherwise he would've unlocked his rinnegan then and there, It only mixed with his own when he unlocked the rinnegan, Much much later. So when madara used izanagi, Hashirama's DNA hadn't merged with his own yet thus he had no senju DNA at that point.
No he didn't he already had Izanagi active and it got released upon his death. He didn't have Hashirama's chakra in until he vomited that piece out and put it into his chest. Sorta like how Itachi put amaterasu into Sasuke.
