Neither of them trained their organs. Tsunade created a jutsu that heals her organs, Kakuzu obtained more organs, Hidan did a ritual, and Suigetsu isn't fazed by puncture wounds.

If Tsunade's heart is hit before she activates her jutsu, she dies. All of Kakuzu's hearts were killed, killing him. Hidan starved to death. And Suigetsu's technique still requires chakra.
