During the end of the series when Madara thought the battle was over he was attacked by Tobirama here:
Much similar to how Izuna died
Madara says this to Tobirama as if he was expecting it.
Madara is no fool as he constantly kept track on who he was fighting and potentially their locations as stated when Sasuke attacked him
With the information provided and the dialogue shown throughout each situation its safe to assume that Izuna was killed because he let his guard down believing Tobirama was defeated much like how Madara was almost killed. In fact if it wasn't for Izuna being killed that same way Im willing to bet that Tobirama would have killed Madara as well.
For the nay-sayers who say "He had sage mode he could dodge"
Its been shown multiple times in the manga that S/T users are near impossible to dodge UNLESS you anticipate where they are going to appear. The fact that Madara had SM only is not a valid reason to say that is why he dodged it simply because EVEN Juubi-Jin Obito struggled to avoid S/T attacks and he is severely faster.
1) This is shown with Minato hitting Obito once then the second time he was caught because Obito knew. (Juubi Jin)
2) Tobirama striking Obito once with Naruto and then stating his FTG won't work a second time because Obito knows he's
tagged. (Juubi Jin)
3) Killer Bee stalemating Minato because he knew his tentacle was tagged.

Based on the information provided its safe to say that Madara survived because:
1) Izuna Death taught him to watch for Tobirama
2) He knew of Tobirama tactics
3) SM did help him but its dodging wasn't the sole reason he survived
Not just because he had SM.
