According to old Kawaki's settei, those tattoos are actually cursed seals (呪印) and the weapon that he's holding is a rod-shape bō (棒状)... We can see from the cover of the first Boruto chapter that Kawaki didn't have those tattoos on his face and then he had it in the battle that happened later... So at sometime he got those cursed seals by some means to get stronger, or something like that? :hmm
