Is this ever specifically stated? I have always been under this same assumption but i cant say with certainty that it is canon. If so then chalk it up to a plot hole

Danzo could use KA with 3T, the same way Itachi could use Tsukiyomi 3T.

This right here, op is smoking. Kakashi is able to shift a transplanted eye from 3t to ms and he's not even the one that activated the ms in it, obito did. Of course you can make the argument that kakashi had it already. But jj madara was able to shift from kakashis 3t to his mangekyo when he stole it wasn't he?

Danzo was never shown with MS activated to my knowledge but Hatake was able to shift stages to the sharingan time an time again..

The kage summit Danzo's eye is covered so we don't know if MS was active.. It's just speculation from all because no1 seen his eye at that moment..
Though if Hatake can do it then it's almost a for sure thing that Danzo can too.

Danzo should be required to shift to MS before he can activate KA. But doing so would mean he has the ability to shift sharingan stages, which a transplanted sharingan shouldn't be able to do
