The Ketsuryugan eyes have some similarity to frog eyes/frog sage mode eyes.

We have already seen a clan be affected by a sage mode area.
The Juugo clan with their passive nature energy absorption and the sage mode area Ryuugi cave.

The Ketsuryugan may have some similarity to that.

Yeah the Chinoike clan with their Ketsuryugan is said to be from the land of Lightning and the sage mode area Mount Myōboku is not, but i dont think this harms the theory.
The Hagoromo clan is also from the land of lightning as well and we have seen those on the battlefield where the Senju and Uchiha battled (during Hashirama and Madara's background story as kids).
People were less bound to certain areas in the past.
People can migrate and split from others before they settled somewhere else etc.

Combinations how the Ketsuryugan may have come into existence:

An Uchiha who split from the Uchiha clan and maybe married a Senju/Uzumaki with frog sage mode abilities.
The Sharingan changed into Ketsuryugan.

Uchiha who lived near Mount Myōboku.
Sharingan changed into Ketsuryugan.

Hyuuga who eyes changed into the Ketsuryugan under frog sage mode/Mount Myōboku area influence.

I think the formula needs to be at least:
1. Already existing dojutsu.
2. Frog sage mode/Mount Myōboku area influence.

The origins of the Ketsuryugan could be in that direction.
As dojutsus and other kekkei genkai dont just appear out of thin air :p.

We know the origins of the Hyuuga, Senju, Uzumaki, Juugo, Kaguya clan (Kimimaro).
We know the Origins of the Rinnegan.

Kekkei genkai have their origins in the Shinju and the Otsutsuki clan influence on earth and mixing with humans.
Possibly new kekkei genkai came to existence like the Ketsuryugan, possibly through examples I have given.
To some extend similar to how a mix of two different colors can form a new color.

Nah.

If having Frog Sage Mode affected the eyes/dojutsu of the offspring then Himawari would have unique looking Byakugan.

Also frogs aren't the only animal to have horizontal pupils. Mongoose, Goats, sheep, and horses also have them.

I think the clan has to have existed before Uchiha was even founded as the Uchiha clan. You should probably just call them Indras Descdndents.

And if not that, anybody that The Sage may have sgared chakra with could have underwent a mutation in my opinion. I dont think they are descendents of Indra, but thats just me.

is could have also been a mutation casued by absorbing nature energy into sharingan,

I think it's just a coincidence that both eyes share horizontal pupils

Yeah I it was just a not too complex but menacing eye to come up with.

However you have valid points.
The eye rivaling EMS in genjutsu.
The eye activated its power through emotional strain.
The eye resembling a Sage mode eye
There were no doujutsu until kaguya came so it definitely originates from her and her kids.

There's no way to know for sure it's origins bc it most likely wasn't thought about that deeply being just a non main story doujutsu. However same could be said about tenseigan but that had a origin too.

It's possible but the flaws in your theory is the eye shows no sage chakra capabilities and the eye is called the blood dragon eye if the eye was really toad related you would think it would give a better hint

But I like the idea that it is some short of mutation
