I think the Izanagi relies on their eyes being intact for it to be used. That said, the only way to guarantee the death of an Uchiha with Izanagi is by destroying their eyes after they're killed, or at least that's what I believe given what Canon presented us.

You can use Izanagi as long as you have a Sharingan available. Using it will destroy your eye regardless of if you have EMS or not.

Both are restored. The eye casting Izanagi is only sealed when Izanagi is over, not during it's casting. If someone is blown up by a Tailed Beast Bomb while Izanagi is casted then their entire body is restored to its previous state, including the eye used for the technique.

No it is not. It is sealed off once izanagi rewrites a reality. The moment it brings you back your eye is white. I dare you post an instance disputing this. You will fail.
