It does make sense because I'm claiming it as a non-worthy feat. Why are you hyping that up when Madara wasn't ready for her attack? "that direction" not Tsunade's direction, so how is it not a surprise attack when neither Tsunade nor Madara knew about it?
It makes plenty of sense because Madara was surprised. It's not like he knew Tsunade was coming. If he did then obviously he would have dodged.
And Tsunade obviously had no idea he was going to be right in front of her yet she was able to give him a kick. Who surprised who? And who reacted first? Because Madara at least got to see

Well Tsunade got stabbed twice when she attacked two different susanoo clones. The larger you are, the easier you are to hit so if Tsunade gets hit to approach something as slow as a susanoo clone, she's going to be casually sliced up against Sasuke who won't even let her get close as he can swing around chidori sharp spear and simply move out of the way unlike the susanoo clones.
Yes she did get ganged up on but the same panel shows that she clearly got out of that situation.

shows how Tsunade was able to multitask against these things and was fast enough to put distance between the clones to not get ganged up again, she was also fast enough to easily get behind one and bring it down. This also shows us that against 10 sharingan eyes she never once faltered throughout that battle to get caught by a genjustu.
