Izanami should work on that version of Madara I had a huge debate on this already lol and as long as Itachi can maintain Susanoo. He should be immune to anything madara has to offer

Kurama gets begged by Totsuka. Not sure what defense Madara has for Totsuka tho Madara best chance is to last until Itachi has no more chakra for Yata which is unlikely.

Forest Emergence and Pollen are enough deadly offense, P.S. other powerful Ninjutsu. Sage Mode would only enhance his already God-like abilities and exponentially amplify his sensing capabilities.
His defense is pretty amazing too, won't be absolutely effective against Totsuka but his layered defenses can hold up enough for him to think of any other thing. Amaterasu will be sensed before-time and cast away if necessary just like Gaara did using his Sand.
Now, Yata is IMO 'meaningless' because Hashi can target from inside the Susanoo. Both have several useful supplementary skills but before Hashirama's overwhelming prowess and arsenal, they wouldn't do much.

So,
Tsukiyomi, Amaterasu and Yata are successfully countered, only Totsuka remains somewhat of a threat...

Now, I'll take into consideration the stamina factor and Hashirama can 'Dance' all day but Itachi will collapse merely after an hour or two and I'm being generous. Even taking out Itachi's sickness factor, Hashirama is a frikkin' Senju, he'd outlast anyone.
