Assuming chakra & jutsu & all the fantasy doesn't exist in the naruto world, it would be pretty lackluster.

Naruto: Diagnosed with ADHD as child. Hyperactive and overly annoying to everyone but easily able to make friends with most people. Pretty successful.
Profession: Social Worker

Sasuke: Extremely cold and diagnosed with sympathy disorder as child. Accepted into top schools and becomes successful but has shady criminal background.
Profession: (I don't even know....uhh businessman?)

Sakura: Chases after sasuke for entire childhood but settles down and lives quiet but productive life.
Profession: Konoha Street Sweeper....jk, I guess nurse again.

Kakashi: Pretty cool guy.
Profession: Teacher
