I think it bcus Kishi doesnt have much time to think about a small and important detail about back pain where logically it will effect Onoki's performance against Madara and his meteors.

But he's an old man now. She can't cure back pain that's cultivated from age and fragility. Honestly Onoki should of died a long time ago, hopefully he isn't alive.
