Well, the MS is the evolution of the Sharingan. While it grants you new, powerful techniques, it makes you blind very quickly. To get the eternal MS, you have to transplant the MS of a blood-related person into your head. This process creates the EMS, which prevents you from the occurring blindness of the normal MS.

I'd have to say Kakashi. He's the only person who used the Sharingan for all 3 of it's abilities.
1. Copy jutsu and techniques
2. Genjutsu
3. Enhanced perception

Sasuke and Madara mainly used his Sharingan for perception.
Itachi stuck with genjutsu.
Obito spammed his MS jutsu like nobody's business.

Sasuke was a Sharingan user with Uchiha DNA when he was 13 years old and that didn't help against Tsukuyomi at that time.

And a blood-relative being the only one being able to break Tsukuyomi means nothing except that Sasuke is the only one that can do it - in no way does it insinuate breaking Tsukuyomi is a piece of cake for Itachi's sibling. And Sasuke did it without even a Mangekyo, which Itachi and White Zetsu already said is close to impossible. Then Black Zetsu said that Sasuke's skill is simply too high, and made that analogy: ''an expert with a rock can beat a novice with a knife".

Kakashi used the 3T sharingan more than any other sharingan user in the anime.. He is the best 3T sharingan user to me.

Madara never showed feats of being a skilled 3T sharingan user EMS yes but not 3T. Itachi was skilled with 3T but his real powers rely on MS so he isn't as effective with 3T as he is with MS and to all those who bring Tsukuyomi to this discussion then y'all need to know that Tsukuyomi can only be used if you have MS z.z Obito was . . . Ehh idk he only had that invisible that wasn't that good and the regular 3T abilities nothing more. Kakashi and Sasuke are the same though I would say Sasuke is more skilled because he showed that he had enough stamina and chakra to keep up against a MS user and win and Kakashi has limited stamina and chakra so he would be useless after some hours of fighting with his sharingan on.

Aside that, kakashi isn't much of the genjustu type, so leaving only Sasuke in the picture and by that I mean the Sasuke vs Itachi arc. He was just good enough that Itachi couldn't help but acknowledge his strength and finally decided that he could now rest in peace.
On the other hand, not much was shown about Madara utilizing the 3T except against Gaara's 4th company as most of his past battles shown were against the 1st hokage who was immune to his genjustu.

Kakashi because he used a 3t more efficiently and mostly in battles through out his life. He isn't seen using gentsu often but he is able to keep and endure genjutsu from uchiha memebers shows his competence.

Holy shit, Kakashi actually has votes when his 3T usage compared to the other three (Sasuke, Itachi and Madara) is the epitome of mediocrity. I will not count Obito as his eye mutated to a 3T Sharingan and then straight to MS. He showed zero 3 T abilities and only possessed it for less than half a second during 2T to MS transition. I am not sure why he is even in the votes.
