I would say Kakashi is worthy of a special mention because he was a truly amazing Sharingan user for a non Uchiha and he only had one eye. That one eye alone granted him the nickname of the copy ninja, who was said to have copy more than 1000 jutsus.

Anyway, if we are talking about sharingan usage prowess this one goes to Itachi.

You're right. One does not break out. Unless they are blood related, which Sasuke is. Manga fact.

On topic, its probably a tie between Itachi and Kakashi. Sasuke showed very few feats besides VotE with 3T Sharingan. Madara just has hype and that one episode in the anime. Obito doesnt have shit with anything but MS lol. Itachi has shown a multitude of powerful genjutsu feats with nothing but his 3T sharingan, and Kakashi was known to have copied hundreds of jutsu with his. We even got to see his 3T Sharingan feats against Zabuza.

Chapter 142: Kakashi vs Itachi. It was back in Part 1, but Itachi does mention that only a Sharingan user with the same bloodline as him could break out of his Tsukuyomi. If I remember correctly, Sasuke demonstrates this later on in their fight. Itachi might restate the fact that Sasuke broke out due to being blood-related, Im not sure. But this is the first instance that the fact is made

He isn't particularly saying that the tsukuyomi can only be defeated with the same blood as his. He's referring to himself, not the jutsu.

That was basically a upcoming setup for the fight between him and Sasuke.

Madara never showed feats of being a skilled 3T sharingan user EMS yes but not 3T. Itachi was skilled with 3T but his real powers rely on MS so he isn't as effective with 3T as he is with MS and to all those who bring Tsukuyomi to this discussion then y'all need to know that Tsukuyomi can only be used if you have MS z.z Obito was . . . Ehh idk he only had that invisible that wasn't that good and the regular 3T abilities nothing more. Kakashi and Sasuke are the same though I would say Sasuke is more skilled because he showed that he had enough stamina and chakra to keep up against a MS user and win and Kakashi has limited stamina and chakra so he would be useless after some hours of fighting with his sharingan on.
