Living. No doubt he would sustain injuries but to say him surviving was only due to PNJ especially seeing as he not only has hashirama cells but access to medical treatment is a bit too much.

Shin himself having Hashirama's cells is questionable, at the very least, especially since he has not shown any physical traits of having them, like the face on his chest, losing control of Wood Release when he's in bad shape (or even having Wood Release), etc.
