That's not the job; the job is to do paperwork and direct the village's future. The only time the Hokage joins a fight is when someone attacks the village that is vastly stronger than the average shinobi.
And how often did such an opponent appear in Tsunade's reign? Once. Once in four years. And that was an unusual occurrence. The same went for Hiruzen; except he had been Hokage since he was a teen and no exceptional shinobi appeared until Obito when he was in his 50-60s.
