I am pretty sure Kishi would have trolled us by showing us that Gai's Alter Ego is actually Kisame. I mean it makes sense considering how Kisame made fun of Gai "With him forgetting faces" and the spiked up hairstyle also matches with Kisame's hairstyle.

Those do seem like things the true Gai would say, but I think it was largely implied that was actually Kisame saying those things to Gai. Considering the comical relationship they had, it would fit. Also, I'm pretty sure you're in a full meditative state when you encounter your true self, so it wouldn't make sense that was Gai's true self if he was still in the real world.
