Tsunade cannot be killed in combat because of her regeneration techniques. War arc Sakura = Tsunade(at the very least) so Sasuke cannot blitz her and end the battle point blank. With no intel Sasuke would definitely be caught off guard by her byakugou and Sakura's yin seal is stronger than Tsunade's at this point in time. It could go either way tbh, Sasuke's genjutsu could easily take her down but she may use her knowledge and find a way not to look into his eyes. She was pretty innovative in her fight with Sasori and she has improved 10 folds since then.
