How did Madara escape death from Hashirama? He used izanagi right? But then why didn't he lose his eye?

It was said (at least i think) that Madara's Rinnegan were his original eyes. And he gave them to Nagato

When he met Obito he had one left eye, so he might have been using Izuna's remaining eye.

Though question about the EMS, so you get that when you take someone elses MS, but my question is, to get someone elses eye, wouldnt you then take out your own eyes and put the other persons in your eye socket? Thus why didnt Madara ust give Izuna his eyes and he get Izunas so then every gets EMS?
