He died in chapter 691, after he lost the bijuu.

Itachi's was pretty brutal, not just the death but the life he lived up until that... being hated and despised by the very people he saved. The pain his illness gave him, even at the end but I imagine it was pretty constant. Having to go through all that, living a "detestable life" all so he could have his own brother murder him.
Add in that he was Edo Tensei'd and was able to see his failure, that Sasuke had fallen so far due (in large part) to his own actions and then to die again.

... poor kid.
