Maddy and Obito made it all happen. Madara before his death and after being reanimated and Obito while he took charge of the Akatsuki and in some other cases.

BZ stood in the sidelines but apparently still influenced and did some other things behind the scenes to make things go his way.

I'd still give some credit to BZ if I were to give some to Madara though since this would've never happened if BZ didn't tamper with the tablet. BZ would've never happened if it wasn't for Kaguya. Kaguya would've never happened if it wasn't for the Shinju.

Because people feared Madara's name. Practically no one knew who Obito was except for some people in Konoha.

Madara also told him to use his name but in the end, Obito was still the one who declared war. Madara never planned for this war to happen. When he was revived, he mentioned things not going according to his plan and when he met with Obito, he began to ask and then said, "Well it's you so you must have something in mind".

He recognized that Obito had taken a different approach. Madara wasn't responsible for the war just because his name was used. It's practically like framing someone except people found out that it wasn't Madara but rather Obito that had done it.
