They needed to gain intel on him, Which she is doing now, the same way she did with Zetsu. His organs are failing though, so the punch is still effected, his life is just prolonged a little.

Go read up the definition of a sucker punch. Sakura literally came from out of nowhere and caught Papa Shin off guard when he was focusing his full attention on Nardo and Sauce.
