I know how many times it failed but does it really need to kill someone for it to be useful?
amaterasu is a really useful jutsu it just happens that it's blocked or evaded otherwise it'll cause so much unneeded gore seriously it one-shoted 8 tails it would've killed it if it didn't have a jin just imagine gyuuki (alone without bee basically the bijuu it self without being inside someone) getting caught in it he'll have nowhere to escape and this applies to many other bijuu's and most of summons.
and it can even be uses to cancel out jutsu since it eats away chakra (like when itachi used it to counter sasuke's katon or sasuke's amaterasu getting through ay's lighting armor thus leaving him with no choice but to cut his arm) and it can be created instantly and hit the target very few can actually dodge it or block it.
combined with kagutsushi it can be taken to a whole other level as like forging weapons from the flames or transforming an existing mass for flames to spikes that can cut through thick ice that even a powerful character's like naruto had difficulties to break out of.
imagine with me that someone got hit with with amaterasu then kagutsushi was used on said flames in the same manner the amaterasu in kaguya's ice dimension needless to say the target would be cut to pieces in an instant.
or for example a susanoo kagutsushi arrow was shot and it hit the target like when sasuke's susanoo arrow hit danzo and in that same moment the arrow turned to spikes via kagutsushi.
amaterasu is quite useful for attack and defense and combined with kagutsushi it gives the user a dozen of weapons to use and add to that the ability to use it while susanoo is activated for some huge scale weapons.
