Uhm... losing in my humble opinion means dying or failing in fulfilling the objective of the mission. I don't really count his first round against Zabuza a loss 'cause he knew he had his team to back him up (Kakashi wanted them to retreat I know but it does not change the fact that Zabuza faced them 1v4 and lost to the collective).
Don't think I can argue much about Itachi or Pain.
