What about titles that were given to people in NV? Is that counted? Like Kakashi of the Sharingan?

If so, then probably Kakashi.

If not, Madara...although he would probably be more infamous than famous, if being famous by being the worlds number one criminal is counted...

Heh, it depends on how what they do in their world translates to "our" world.

Naruto could be a famous public speaker, while the Akatsuki could be a famous criminal organization, Kakuzu could probably become very rich, Shikamaru or Shikaku could win those game/trivia shows or be a military strategist (depends on what kind of intelligence you mean), most of those ninjas who are considered hard-working or the best in their fields would be the closest to the top of their careers. And I guess the Kage could be real life politicians.
