Gaara's gourd sand is high in both quantity and quality, so that + desert suspension should be enough to shield Gaara from all of Itachi's techniques. Third eye + suna sensing counters clones and 3T genjutsu, while Gaara gradually makes enough sand to overwhelm Itachi via Sabaku Taisō or Sabaku Sōtaisō if he brings out susano'o. Itachi isn't maintaining susano'o for long and Gaara has very high chakra reserves, so simply outlasting is an option as well.

Gaara wins even with Tsukuyomi and Amaterasu. The sand is an excellent counter to the Sharingan's abilities, given its incredible defensive capabilities. Itachi's only real chance is Susano'o, and Gaara was seen literally ripping Madara out of his Susano'o, which suggests to me Gaara would have the upper hand even there.

Uh, how is it not? Third Eye hard counters visual genjutsu by obstructing direct eye contact between Gaara and Itachi. Even it doesn't, sand sensing makes genjutsu completely useless in this fight. Susanoo slashes are tanked with ease (

The Third eye is connected to Gaara's optic nerve, and that's how Ocular Genjutsu hits it's targets.

Third eye dosen't stop genjutsu since it's still a direct connection to the user.

Hence why garra was still under Gengetsu's genjutsu despite eye being out. Sensing sand work's tHo.

and either way about the sand eye, Itachi will never be in position to use MS genjutsu against gaaras style of combat, w/ its range mobility & perpetual pressure tactics.
