What did Oro mean by straining?
When do old people strain? When they have a heart attack or multiple organ failure.
Hiruzen was straining because his organs were literally failing because of old age.
He could have dropped dead at any moment.
Hiruzen was already on the verge of death and had he not used RDS quickly, he would have died naturally in the middle of his battle with Oro.
See, that is how handicapped Hiruzen was against Oro. It wasn't fair at all.
In edo form he was able to do cancel with Shinjushenjuu, he didn't show this scale at all against Oro because his organs were failing at the time.
That is why Edo Hiruzen showed much greater feats.
He was brought back in a state where his organs weren't failing. His edo form was probably based on Hiruzen's body when it was a few hours before the Oro invasion.
Regardless, the huge descrepancy between Edo Hiruzen and old Hiruzen shows that there was something wrong with Hiruzen during his battle with Oro.
With everyone else, the living form is stronger than the Edo form. With Hiruzen, it's the opposite. His Edo form was stronger than the form he showed with Oro. Then that means that there was something wrong with Hiruzen. And I'm sure that it means he was dying already during his battle.
It's kinda like how Edo Itachi is stronger than Itachi during his battle with Sasuke because he was already dying during that battle.
Edo Tensei brings a person back from around the time period they died minus a few days, but it doesn't bring them back from the exact moment they died. Otherwise, Edos would be useless.
