Remember when sasuke landed amaterasu on blind madara? why didn't sasuke use kagutsushi on the flame on madara's back and cut him to pieces with enton spikes? he was just standing there and watching instead why didn't he use it?

Amaterasu was being absorbed then. You see, when preta path is in use there's a circular envelope in which anything chakra based that bypasses it is immediately absorbed and it loses its effect. Therefore manipulating amaterasu/enton within this region is not possible.

Here's an example:

Using Preta Path forms a BOUNDARY around the user, and so anything that goes past it is IMMEDIATELY ABSORBED and rendered ineffective. You can see this BOUNDARY here:

Notice that as soon as it PENETRATES the CIRCULAR ENVELOP it immediately DISFIGURES and is loses its EFFECT

you may be right but sasuke could've used kagutsushi from the start before madara absorbed it and physical damge from jutsu was shown when used on preta path users before mainly when sasuke cut jj madara in half and when naruto used magma rasenshuriken on him.

Whatever Sasuke used, madara like nagato can use shinra tensei or preta path to negate the jutsu. Then hashi's cell would heal him from any damage obtained​

madara healing is kinda slow that's why he replaced his arm with a zetsu spare instead of waiting for it to reform not to mention that kagutsushi would mostly do more than scratches as see here:

it managed to make alot of cuts in a moment so it would most likely cut him to pieces than injure him and shinra tensie couldn't happen since he does not even have one of his eyes and if it does I doubt he'll make it in time.
