Madara doesn't, but then the user goes on to say that there is logic and science behind not being able to do it. Sure, Kamui was restricted because it's too much chakra to constantly phase, but wood-style clones are nothing like that.

It's 4 or 5. The DataBook says 4, but then in the Manga, he creates 4 with the 5th seemingly still captured by Naruto. If it's 4, then that means Limbo cannot be sealed.

Yes, that's how it works. I couldn't tell you the exact reason, since I'm not Kishimoto, but phasing is restricted, while teleporting in whole is not. Perhaps it's the fact that Obito could not tame the ten-tails, therefore phasing would be impossible with such large reserves. Here is the official page from VIZ:

That's close to my hypothesis, Madara seemed to have greater control over the power of the Juubi(he didn't even go through a struggle like Obito did). Plus Madara having a stronger body with senjutsu also allowed his body to handle the power better than Obito.
