I'm pretty sure Obito sucked lot's of fodders with Kamui to another dimension and killed lots by using it to teleport. Kakashi cut Deidara's hand with Kamui (it's just that Kakashi did not have enough stamina to cut his head). And by the way, Kamui is not the technique that would be used to kill, rather to teleport and take advantage in the battle, while amaterasu is a technique that you direct and kill with.

Kamui can be used to kill; just not directly. suck you into the Kamui world and leave you there to starve.
