Madara basically had his eyes implanted into Nagato when he was a kid, to keep them safe. Madara unlocked the Rinnegan when he implanted Hashirama's DNA into a wound on his chest, after he used Izanagi to alter reality and bring himself back to life after his defeat at the hands of Hashirama. Nagato was able to wield the power of the Rinnegan effectively because he was an Uzumaki, and only an Uchiha with Senju DNA or a Senju (And by extension Uzumaki who are a side branch) with the Doujutsu of an Uchiha can attain and use the Rinnegan.

Now, for the question noone cares to ask.
When was Madara traveling through the land of Rain. Encountered a young Uzumaki child. Take out his eyes, gave him rinnegans. Than disappeared to a tree near konoha and nothing went wrong.

That whole scenario seems a little far fetched as we know he used the eyes to kill his parents attackers so he was very young.
I feel like they already gave pain the rinnegan and through storyloops had to make the rinnegan a success to the sharingan so somehow had to relate it to madara.
