Remember when he used it on gaara, his body was under so much strain that he didn't notice gaara swapped out of his body, he didn't show any signs of strain when he used it on sasuke.

The only reason he was under stress is cuz garra is heavy. He had to over exert himself to get garra to the proper altitude to perform dancing leaf shadow
He had 0 problems with sasuke.

Ninjas in NV smash into the ground all the time without sever injury. Lee's technique only has more force behind it but it shouldn't kill him unless he falls flat on his head.
