Stuff happens off-panel as well. Perhaps Konan expressed her concerns about Obito, but Nagato simply didn't acknowledge the threat. Do keep in mind that Tobi didn't show many feat to be a threat to Pain/Nagato, or Nagato was under the impression he could outsmart Obito.

Konan simply was smarter than Nagato (don't believe Nagato was the most intelligent in the series by far) and saw the threat coming.

About the Madara-Sasuke thing... Do keep in mind that anybody telling Sasuke about Madara was massacred and Sasuke might have visited the shrine in between these events (didn't he?).
