Sharingans are said to be the eyes which reflect feelings, so if an uchiha experiences a strong emotion like danger, hate, and other feelings the regular eye mutates into what we know as sharingan, then again an uchiha is said to awaken the MS when the sharingan wielder goes through such a terrible pain that the eyes in response mutate into MS, that terrible pain is in all times we´ve seen caused by the deaths of somebody they deeply love.
Itachi didnt kill shihui but he did presenced his suicide and awakened the MS, sasuke saw itachi dying, Madara and Izuna were said to kill both their best friends but i doubt that since madara and izuna deeply cared for the uchihas, so i´m asumming that their best friends died in war and so they awakened their MS, kakashi and obito activated the MS at the same time, kakashi by killing the person he most cared for and also the one he sworn to protect, and obito presencing the murder of the most imprtant person in his life by the hands of his best friend causing both of then almost unbearable pain, as for shihui is unknown how he awakened his MS but it must´ve been somthing similar.
My point here is that so far the only people we truly know awakened MS were the ones mentioned above, but also it´s hard to believe only them of all the uchiha were the only ones who saw a comrade they truly love and care for dying before their eyes, in that case shouldnt most of the uchiha awaken their eyes when a relative of theirs die, assuming they already awaken the sharingan before of course? i mean almost all people go through the pain of seeing somebody they love dying why only these few uchihas were the only ones who awakened MS? shouldnt there be more?
The only explanation that more or less fix this problem is the one of tobirama, tobirama said uchihas love way too much and when uchihas lose that love they go nuts and some of them awaken the MS and that love turns into hatred, but that concept shouldnt apply for all uchihas only a few ones that are more sensible, so in conclusion Itachi, Obito, Madara, Sasuke, Shihui and Izuna they basically awakened the MS because they loved more than any other regular uchiha, their love was way too much to be contained and it eventually exploted and became hatred in the form of MS´s.
