OK, having any tailed beast extracted kills you, FACT. Gaara had his tailed beast extracted, he died, ok, following the rules set in the manga. BUT, Chiyo brought him back to life.

Thanks to Sakura giving cpr giving oxygen and pumping his heart she was able to stabilize his body with a oxygen and blood flow, until Minato could give him the other have Kurama's chakra, and when he did, Naruto is back to life.

Here's the deal, Gaara,Gai,Naruto and Obito survived because the plot required them to, as for Madara's case even if he lives what will he possibly do? The plot requires him to die, and that's why you can be kinda sure he will die or is probably dead already.
