Madara wins the first scenario. He is faster than Itachi, he is physically stronger than Itachi, he's most likely more durable due to the fact he's almost a middle aged man and has armor. He's also blocked Ay's hit, which even KCM Naruto noted was extremely heavy.

Second scenario is a stalemate since they can't fool each other with Genjutsu.

Last scenario is a stomp in Madara's favor. His Katons completely dwarf anything in Itachi's arsenal. He isn't going to evade them, nor is he going to block them. So he dies with the first or second move.
