Madara is the only one to die three separate times.

He died when he lost the VOTE battle against Hashirama, even though he brought the strongest tailed beast with him. He died again, old and all alone in a cave with none of his goals or plans for the future being met. Even after that, when Obito has done everything he couldn't (Starting a war, reviving the Juubi, Uniting the shinobi world against him) he still manages to die a third time.

Madara has managed to die 3 separate times after coming back to life, 300% more than anyone else.
