Tsunade's ass let Naruto go after Sasuke, even though there was a high chance of him dying and the Kyuubi getting released.
She also should have went after Pains ass as soon as she heard about the invasion.

It was foolish on Tsunade's behalf to send a bunch of Genin on a retrieval mission against high-level ninja, each of their lives were almost lost. However, we do have to remember that Tsunade knew she wasn't fit to be Hokage and never wanted it in the first place and wanted Jiraya instead so... I'm going with both lol

She is one of the most skilled taijutsu fighters of the manga, combined with unholy strength, extremely high chakra reserves, battle immortality, several high level medical techniques ( chakra scalpels, ranshinsho ), poison and with a boss summon who is immune even to the village buster techniques, such as the Cho Shinra Tensei.
