This is Kakashi's eye sight when he's using Kamui

he use it on what he sees and he sees Susanoo. He can see Itachi but the Kamui would affect were Kakashi's vision is which would be the Susanoo, that would be his target since you can't look at someone and complete ignore what you're looks at. How can you focus on Itachi when Susanoo is covering him? That would only work if he first kamui Susanoo then Itachi. Susanoo is like a tint Window like armor and as you can see in the link, he focus on a point which he can't do on Itachi since Susanoo is still there.

You do know that is Sasuke using Amaterasu and not someone using Amaterasu on Sasuke from outside Susanoo. If Kakashi could warp beyond a barrier or shield then he would have to the Gedo Mezo when Obito put it in a barrier; after it was in a barrier and Bee couldn't break it, they ignored it and started to fight Obito and Madara and Kakashi obviously knew his power couldn't do that even after Kurama gave him more chakra.

Sasuke was already inside of the ice to begin with, and inside Susano'o [His own technique]. There is no evidence of someone igniting flames where they cannot see or beyond an obstacle, which you can then in turn use as evidence of someone being able to use Kamui where they cannot see.
