A possible explanation :

Yamato is the only living person who is capable of using Mokuton Jutsu after Hashirama Senju.
Yamato must have been used by the Hokages before as well. (check out danzo's quotes in middle panel)

So, we can consider that Yamato was given the name "Yamato" by the previous Hokages as well and there wasn't any change in the name.

Here's the main proof for this so called MISTAKE we all are talking about which can prove Hiruzen and Yamato had connection previously.

These all scans proves that there was a huge connection between these guys with Yamato. And this since Hiruzen called Yamato as "Yamato" means that the codename was used common by the previous Hokages as well.
