I have already said sasuke can teleport people near him, This thread however explains how he used hid ability with the sword

Sasuke just threw the sage-chakra-infused-sword towards limbo madara and as limbo madara got hit by it, the real madara took the damage.

Sasuke simply dodge Madara. The Limbo returned to Madara body. Sasuke explained this. Madara blocked Naruto's attack with Limbo, but his arm was damaged.

Nope, not completely right. If Sasuke would just change places with his Katana, then it wouldn't have stabbed Madara in chapter 674. To do so, he had to change also the position of the Katana.

Sakura's vest was beside her while sasuke was in the desert dimension

Yes, I believe that he just teleported that sword into Madaras body. Like I said, if he can teleport his enemies then I dont see a reason why he wouldnt be able to do the same with his weapon.
