The Entire Uzumaki clan.

Simply because there was no clan in Part 1.

There was nothing in part 1 that hinted at Naruto's last name bearing any relevance to others.
But then, all of a sudden in Part 2, it's a major clan that allied with Konoha.
Konoha even reps their symbol on the back of their vest.

But not ONE person, in the entire village of Konoha recognized Naruto's last name.
Not Zabuza, who grew up in Wave country which was near Uzushiogakure,
Not Tazuna who should be old enough to have heard about the Whirlpool Ninjas
Not Sakura who was supposed to be real smart and knowledgeable on a bunch of pointless things
Not Shikamaru who was claimed to be a genius
Not even Choji who has Naruto's clan's markings on his cheeks for some reason.

Tsunade didn't even think about it when she met him.

It was BS how Kishi just suddenly created the clan in Part 2.

Lol If you didn't realize by now, a lot of things were kept hidden from the public eye in this series. It wouldn't be surprising that something like this was covered up in the ninja world. This isn't even a plothole, just a plot convenience.

Why would Tsunade, who had no contact with Konoha for at least 15 years, cover up her relation to Naruto, or not even remark on it?

The same ploy Jiraiya and everyone else used. The Third Hokage did have everyone stay quiet about everything after Minato and Kushina died. Plus the fact (and this may be off topic) that she was rarely in the village after the Great Ninja wars and barely got to know anyone, maybe even Minato and Kushina.

The biggest plothole?

A several-year-old Kyuubi Jin living alone in Konoha with no one watching over him whatsoever.

Kamui in -> Show Naruto some ramen -> Kamui out -> Keep the fox in a jar in a basement until other beasts are captured.

But Tsunade left like 20 years before the series started. She wouldn't even have known that Naruto shouldn't be told anything.
But actually, she did know that Naruto had the kyubi. The fudge? Did she like exchange letters with Konoha or something?
