Tsunade lost the people most important to her.
And I mean, she was already known as the medical expert and still failed to save her lover. I'm sure that makes you blame yourself at least some amount.

Didn't Orochimaru's parents die when he was a child or something? I'd say Tsunade's past is worse, since she lived a pretty good life until then, even if during wars. Then suddenly her happiness was just taken away and she was left without anything.

I honestly completely forgot about that.
And I disagree. Your parents dying at a young age is worse than losing your younger brother and lover.

I still disagree though, she was trolled by life big time. First giving her everything and then taking it away in an instant.
Orochimaru grew up coping with the fact that he lost his parents. Like Iruka for example. :|

I'd actually be sadder if my sibling died compared to my parent.

1). Madara/Hashirama - Their families and friends were brutally killed in constant war.
2). Kakashi - His father committed suicide, his 2 best friends, thus breaking the promise he made, and found out his best friend lived and became evil.
3). Kabuto - Never knew who his parents were, ignorant of almost everything, and was manipulated by everyone.
4). All Jinchuuriki - Hated for carrying monsters.
5). Nagato - Manipulated by the evil Madara, his parents were killed, starved for days, and his best friend was killed.​

I'd actually be sadder if my sibling died compared to my parent.

But as a young child. Imagine the scar and the void. Him losing his parents at such a young age and then seeing the fragility of human life drove him to madness.
He was, what, six? Five? Not even thinking about death at that age, then it becomes a major part of his life in the most traumatic way. I can see where you're coming from, though.
