2) Suppose it did happen. Shisui didn't want to kill Danzo. That's why he put him in a regular genjutsu, then dipped. If he wanted to, I'm sure KA followed by decapitation would do nicely.

3) Izanagi served as a surprise element here. How was Shisui supposed to know Danzo could use a forbidden Uchiha tech? If he knew he had Izanagi, Danzo would be Done-zo.
