Throughout Naruto people have ripped peoples eyes out of the socket.
I want to know what was different about the time when Itachi was "trying" to take Sasuke's eyes,
because he's the only one to have showed any pain (he showed a lot) and everyone else didn't.
I mean shit Kakashi looked like he didn't know what happened. So whats different?

Itachi ripped Sasuke's eye out while in Tsukuyomi, the genjutsu probably made it feel like it hurts worse than it actually would (especially for a ninja who can take great deals of pain).

Adrenalin negates pain and you could tell Sasuke was crapping him self and Kakashi is in a fight adrenalin is going to kick in

I don't know it was never stated that he could control what a person feels just the time a person felt in a jutsu.

well, its supposed to hurt in real life and in manga when you ripp out a eye right out of your brain

I'm really surprised about this. I mean, it's ridiculously easy to take one's eye out or implant it in your eye socket. Come one, I have trouble putting my contacts in or taking them out, imagine the actual eye.

At the Uchiha Hideout, Itachi had cast Tsukuyomi on Sasuke, hence the reason why it hurt Sasuke a hell of a lot more than any other Shinobi whose eyes were torn from their sockets... an eerily common occurrence throughout the series. I mean, just look at Kakashi in Part I under the effects of Itachi's Tsukuyomi... the man was in trauma for weeks as a result. Sasuke narrowly escaped Itachi's torment with his own Sharingan at the time Itachi "tore" his eye from its socket. So, yeah... it's simply a matter of pain-inducement genjutsu vs the reality of Shinobi in the Narutoverse who are more than capable of enduring a load of physical pain.
