Sasuke isn't attacking from a blind spot, kaguya has the byakugan so she can see nearly 360' right now. Sasuke isn't attacking from the blind spot so kaguya has no reason to actually look up at him since she can still see him.

Now if naruto was in the same situation he would be fine. His sage mode sensing plus negative intent sensing would alert him and with his reflexes he would simply dodge or counter. Naruto's clones are able to fight limbos he can't even see, and naruto overwhelmed madara who effortlessly countered ftg another instant s/t jutsu. The manga doesn't really do it justice but the fights at the moment are happening at ungodly speeds.
