Now , take a look , what was she back then? Pretty much nothing due to her fear of blood and kabuto easily beating her etc

She only went ahead and beat up orochimaru after she overcame her fear due to Naruto's bravery , so if Hiruzen and Tsunade were to swap placces back then , Orochimaru just need to bite a thumb , smear blood on his face and Tsunade will be rendered useless
