He does not have CoAT because :

1.If he had,he would have created rinnegan/sharingan instead of a regular eye.

Sage used izanagi to "create" . He needed to use izanagi to use creation of all things. To use izanagi , one must have sharingan(or its upgraded version, rinnegan) . Naruto does not have both sharingan and rinnegan.

The image given in OP clearly states, izanagi was used to create.
Not CoAT was used to use izanagi.

This also implies sharingan/rinnegan is necessary to use CoAT.

I think madara would be able to use it after implanting the second rinnegan in his eye socket.

Are you sure that scan you're using is the viz? Izanagi is a jutsu based on coat. One needs great mastery over yin & yang to achieve it.... However i do agree with naruto not having it.

It does use yin-yang release and so does creation of all things... but the difference is, it was found after COAT was used.... it is a derived jutsu unique only to senju-uchiha dna users. But the principle of creation of all things is simple. If you can simultaneously use yin-yang release, no uchiha or senju crap the creation of all things is possible.

However, you mastery had to be SO6P's level or near it...

One has to know how to use CoAT to derive a jutsu from it.

Amaterasu is a parent jutsu and Blaze release was derived from it.
Rasengan is a parent jutsu and FRS/variations were derived from it.

So if we follow your logic, you are implying izanagi was derived from CoAT, which will further imply that one has to have the ability to use CoAT to use izanagi. This is in fact false assumption as many people have used izanagi with absolutely no knowledge of CoAT and how it is used.
Itachi explain how izanagi was used by many uchihas but,obviously, it is impossible for them to have the ability to use CoAT.

CoAT is basically a next level of izanagi, like FRS is next level of rasengan.
There are obviously a lot more elements involved in CoAT but izanagi is one of the basic requirements.

FRS is a variation. Izanagi is not a variation. it is a completely different jutsu that uses the principle of COAT.

COAT is not the next level. it is something different. SO6P was said to have been able to create anything out of nothing and breathe life into it. Thats why the bijuu have a conscience and a supposed living body rather than just being masses of chakra.
