Eternal Mangekyo Sharingan

There were rumours that Madara killed his brother in order to take his brothers eyes, and obtain the Eternal Mangekyo Sharingan by implanting them within himself.
Madara says that Izuna left him his eyes, in order to further protect the Uchiha clan.
Sasuke Uchiha killed Itachi, and then went on to implant Itachi's eyes, in order to obtain the Eternal Mangekyo Sharingan. This is less relevant to my point though, as Sasuke killed him for ulterior motives.
Why is it not possible for brothers to trade eyes, for example, could Sasuke and Itachi, had they have been on better terms, give eachother their eyes, and both progressed to the Eternal mangekyo sharingan?

To be performed, the eyes of the donor sibling must have been removed, preferably with a medical technique so as to preserve their integrity. To me that doesn't really suggest a fusion of eyes. It says the eyes change host, that could mean being simply implanting them in place of the other brothers eyes.

I'm talking about the monster thing...i mean it have to be a reason they showed that with all 4 eyes for EMS

But that's wrong. Actually' it's possible just to trade the eyes, but in any case their is no warrant that anyone would obtain the eMS-according to Itachi.
The statement that the two pair of eyes have to fuse can't even be right, because Madara, Izuna and the whole Uchiha-Clan didn't know anything about the eMS, before Madara took the eyes of his brother. He was the first one who could lift the secret about the eternal light.

Well, if we think about it carefully, the chakra of both-of the old and new owner- should be the reason why the pattern changes.
