Pretty sure it's not a simple case of swapping Eyes.

My guess is they use some forbidden Jutsu to Transfer/combine the ocular powers, After that is done, The Eyes that were taken out are destroyed/lose their powers

For Madara and Izuna, I'm sure they were unaware of EMS and Izuna was going to die anyway.

As for Itachi and Sasuke, Itachi wanted Sasuke to kill him so he could be the one to avenge the clan. He wasn't truly concerned about EMS at all. If anything he wanted for Sasuke to have it.
